A donation form can be found on each Priority Health 5K Walk brochure. You can also call 269-345-1913 to receive a donor packet. Or, click and print your own donation form. You can turn in this form and donations when you pick up your packet and t-shirt. (Packet pick-up will be at the Borgess Health & Fitness Center on April 27 from 4-8 p.m. and again on April 28 from 6:30 - 8:15 a.m.) You can also mail in your donations anytime before April 28 to: Borgess Health & Fitness Center, 3025 Gull Road, Kalamazoo, MI 49048.
